You may know that you or a loved one needs help, but may not know exactly what type of care is needed, or even what is available. Our experts can help guide you through this process, by evaluating you or your loved one. We complete a thorough assessment to gain a full understanding of the issues and concerns faced by the client and caregiver. Our registered nurse case manager along with a geriatric social worker will assess the senior's needs as well as the family members involved and recommend appropriate guidance to ensure that your loved one is safe and secure. The assessment will target options that are available and that best fit the client/caregiver relationship and include:

Our home health aides can help with:

  • Medical history of the client
  • Social and psychosocial issues
  • Needs for financial and legal planning
  • Caregiver support needs
  • Home safety issues

You will receive a written report, which will detail our individualized findings and recommendations to ensure that you or your family member is safe and secure. Based on the suggested plan, you can decide what steps you would like to take next.

Assessments Required for Entrance into a Skilled Nursing Facility

Before entering any skilled nursing facility, a Patient Review Instrument (PRI) is required by law. Our nurses can properly assess your medical condition to determine whether you can currently function safely in the community or would be more comfortable in a skilled Nursing Facility. Our expertly trained nurses can perform a PRI to determine the best type of nursing facility for your needs. Day and evening appointments are available 7 days/week within 24 hours of a request. A PRI can determine:

  • Medical condition/diagnosis
  • Medications needed
  • Special diets
  • Therapies needed
  • Physical and mental abilities and limitations
  • Ability to peform daily tasks
  • Behaviors that may need management, such as aggressiveness, anxiety or depression
