It happens so fast. One day a person is healthy and active, then you begin to notice the changes.

Suddenly, your older family member needs help with everyday activities. You want to help but you can only do so much. It can be difficult to handle the medical, physical, social, emotional and financial issues that come with caring for an aging adult. What help is needed? How will you obtain it? Anchor Inspiration Program can help! We offer a wide range of services that are specially designed to provide peace of mind to seniors and their caregivers.

Getting Started

We begin with a thorough assessment to gain a full understanding of the issues and concerns faced by the client and caregiver. A Registered Nurse Case Manager along with a Geriatric Social Worker will assess the senior’s needs, and the needs of the family. This care team will recommend appropriate guidance to ensure that your loved one is safe and secure. The assessment will identify options that best fit the client/caregiver relationship. Your At Home...assessment includes:

  • Medical history of the client
  • Social and psychosocial issues
  • Needs for financial and legal planning
  • Caregiver support needs
  • Home safety issues

You will receive a written report, which will detail our individualized findings and recommendations to ensure that your family member is safe and secure. Based on the suggested plan, you can decide what steps you would like to take next. You may prefer to do it on your own or utilize the many resources that At Home... can make available to provide you with peace of mind.

At Home... is an individualized Program that can be customized to meet all of your needs. You can use one or combine any of the services below to meet your needs.

Case Management

Our registered nurse case manager can provide on-going coordination and management of your loved one’s care needs. Our case manager can help families and individuals cope with issues and decisions associated with caring for the aging. These services can include scheduling of appointments, communication with physicians and education about medications. In addition, our social worker can provide on-going support relevant to evaluating and selecting appropriate housing options including assisted living facilities, nursing homes and other senior residences. Linkages to community resources such as Adult Day Care and Meals on Wheels are also available along with bill paying services. Our social worker can also assist with preparation of Medicaid applications when appropriate.
